In making this application for enrolment, I am aware of the consequences that may arise from providing false, misleading or incomplete information, including the cancellation of my enrolment or the withdrawal of any offer made by Sydney Design School.

I am aware that Sydney Design School is required to submit data sourced from this enrolment form to the National Centre for Vocational Education Research Ltd. (NCVER) as a regulatory requirement. I have read and I consent to the collection, use and disclosure of my personal information (which may include sensitive information) pursuant to the information detailed, and NCVER policies, procedures and protocols published on NCVER’s website at www.ncver.edu.au

I am aware that the information contained on my enrolment form may be used by my RTO or the following third parties for administrative, regulatory, and/or research purposes:

• School – if I am a secondary student undertaking VET, including a school-based apprenticeship or traineeship
• Employer – if I am enrolled in training paid by my employer
• Government departments and authorised agencies
• NCVER
• Organisations conducting student surveys
• Researchers

I understand that I may receive an NCVER student survey which may be administered by an NCVER employee, agent or third-party contractor and that I may opt out of the survey at the time of being contacted.

Privacy Notice

Under the Data Provision Requirements 2012, Design Schools Australia Pty Ltd trading as Sydney Design School is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER).

Your personal information (including the personal information contained on this enrolment form and your training activity data) may be used or disclosed by Sydney Design School for statistical, regulatory and research purposes. Sydney Design School may disclose your personal information for these purposes to third parties, including:
/ School – if you are a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
/ Employer – if you are enrolled in training paid by your employer;
/ Commonwealth and State or Territory government departments and authorised agencies;
/ NCVER;
/ Organisations conducting student surveys; and
/ Researchers.

Personal information disclosed to NCVER may be used or disclosed for the following purposes:
/ Issuing statements of attainment or qualification, and populating authenticated VET transcripts;
/ Facilitating statistics and research relating to education, including surveys;
/ Understanding how the VET market operates, for policy, workforce planning and consumer information; and
/ Administering VET, including programme administration, regulation, monitoring and evaluation.

You may receive an NCVER student survey which may be administered by an NCVER employee, agent or third party contractor. You may opt out of the survey at the time of being contacted. NCVER will collect, hold, use and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at https://www.ncver.edu.au/).

We are required to assess the suitability of your chosen course accounting for your skills and competencies. If we believe the chosen course is not suitable for you, we will contact you to discuss prior to enrolment.

• Digital literacy: you will be required to access resources by navigating our online platform, manipulate documents including scanning and converting to PDF, and communicate via email. Software courses require a high digital literacy to understand program requirements and procedures.
• Numeracy skills: you may be required to complete tasks such as converting measurements.
• Literacy skills: a strong written comprehension is required to understand course materials.
• Language skills: you may be required to present information and projects in a written or verbal format.

Examples of how you can demonstrate these skills include:

• Previous studies
• Workplace skills
• Regular computer use
• Ability to troubleshoot

/ I have read, understood and agree to the Terms and Conditions on the Sydney Design School website including information regarding VET Student Loans for the Diploma and Advanced Diploma only.

/ If I am under 18 years, I have had a parent of guardian give their permission by signing this form.

/ I have also read the Student Handbook.

/ I understand that I will receive a credit transfer on the units of competency I have achieved while studying a previous course at Sydney Design School.
